The UDID Card refers to the Unique Disability ID Card. This is an initiative by the Department of Empowerment of Persons
with Disabilities (DEPwD), Ministry of Social Justice and Empowerment, Government of India. The UDID card provides a common
identity for persons with disabilities, streamlining the provision of benefits and services. It includes a unique ID number,
personal details, and disability-related information.
The Unique Disability ID (UDID) card allows physically handicapped individuals to access a 3% quota in government jobs and
benefit from various government schemes. The UDID sub-scheme aims to create a National Database for Persons with Disabilities
across the country.
To apply for a Unique Disability ID (UDID) card, you can:
Email Address – disability-udid@gov.in
- Go to the UDID portal
- Click Apply for Disability Certificate & UDID
- Enter your personal, employment, identity, and disability details
- Upload the required documents
- Click Submit
- Download your E-UDID card
What happens after you apply?
- The applicant sends the application to the Chief Medical Officer (CMO) or District Medical Officer (DMO).
- A specialist doctor assesses your disability
- A medical board reviews the case and assigns a disability percentage
- The CMO prepares the Disability Certificate and UDID
- The authorities print and send the UDID card to you.

Documents required for UDID Card
Applicants must submit the following documents to apply for a UDID Card:
- Scanned copy of a recent color photograph
- Scanned copy of identity proof
- A scanned copy of the address proof
- Scanned image of signature (optional)
- Scanned copy of Disability Certificate (available only to people with disabilities for whom the appropriate authority has issued a certificate of disability)

Types of UDID card
There are three types of UDID cards. They are as follows:
- White Card: This card is issued if the beneficiary’s disability is below 40%
- Yellow Card: This card is issued if the if the beneficiary’s disability is above 40% but on or below 80%
- Blue Card: This card is issued if the beneficiary’s disability is above 80%
Validity of UDID card
There are two types of validity for UDID cards:
- Permanent UDID cards and Temporary UDID Cards.
- Permanent UDID: If there is no progress or regress in the disability of a beneficiary, this card is issued by the medical authority.
- Temporary UDID: If there is progress or a regress in the disability of a beneficiary, this card is issued by the medical authority.
